73 year old with severe, bilateral bone-on-bone knee arthritis. Progressively worsening pain for the last 10 years. Pain worse with walking, standing, stairs and at night. She has been using a cane for walking for the last 5 years. Previous cortisone injections provided brief relief.

Her X-rays showed severe bilateral arthritis with bone on bone appearance in both knees. MRI imaging of the left knee revealed some areas of swelling and inflammation inside the bone of the femur and tibia indicating arthritis changes in the bone itself. As arthritis progresses and you lose more and more cartilage shock absorber, the bones become under increased stress and can suffer. Eventually, with continued weight bearing stress to the joint, degenerative arthritic changes occur within the bone itself and can cause notable pain.
